How does someone 320lbs get health insurance?

I live in the Baltimore area and am out of work at present time. I am seriously considering weight loss surgery, however I don't have health insurance and cannot afford to pay out of pocket. Does anyone know if certain insurance companies HAVE to accept you? I thought I remembered hearing this about HMO's. Please, if anyone has any suggestions please email me at [email protected]
