has anyone ever been refused insurance due to their weight

I was denied insurance because of my weight they told me that I was a high risk and referred me to the california major risk medical insurance program MRMIP it's going to cost me 333.00 dollars a month and I chose Blue Shield PPO they have a 90 day pre-exsisting condition exclusion then after the 90 days they say that they will pay for pre-exsisting conditions does anyone know if weight loss surgery will be paid for.
