Approved by ins, new book left out except when medically necessary

My company's re up date is October 1. I am scheduled for surgery this month and just received my new cards and new book. They used to exempt WLS except when medically necessary. The new book leaves out "except when medically necessary". Can they change their minds after approval. Will I have the surgery and then get denied and be stuck with the bills. Help
