What happens if you can not show 5 years of medical records?
I went through Kaiser and we did not need to provide 5 years of medical records. The main requirements for anyone within Kaiser that wants WLS is attending a mandatory seminar, mandatory visit with the doctor in charge of wls, mandatory 26 week Options class at Kaiser in Orange or another approved Kaiser facility, mandatory seminar with the medical group contracted (Pacific Bariatrics in San Diego) with Kaiser, and mandatory physc/internal medicine and surgeon aproval. If approved by Kaiser, the total cost is paid by Kaiser with the exception of your co-pay or deductiable. In my case, it was $100.00.
To answer your question, I don't see how you can be denied if you meet the requirements.

SAMPLE REQUIREMENTS:
- Generally a patient must weigh a minimum of 100 pounds over their ideal body weight, The ideal body weight is determined by standards of height and weight (BMI).
- Failure to lose weight despite documented participation in at least one formalized weight control program.
- There must be a medical condition directly caused by, or made worse by the extra weight, such as:
- Hypertension
- Cardiovascular disease
- Diabetes
- Sleep apnea
- Chronic respiratory insufficiency
- Dependant edema
- Chronic degenerative joint disease of knees, hips, ankles or back
- (including gouty arthritis)
- Pickwickian syndrome
- Severe gastroesophageal reflux disease
- Chronic depression as a result of obesity
- Hyperlipidemia (elevated cholesterol or triglycerids)
- Gallbladder disease
- Recurrent incisional hernias
- Chronic phlebitis and/or chronic venous insufficiency
