Insurance Help

My insurance will change to Empire Blue Cross Blue Shield. I was wondering can anybody tell me what requirements they have and also I am confused because my new  ins is from New York Help Please. If anyone can tell me how this ins is on approving surgery.

you can do two things.  go to their website or else call them.  It is cut and dry as to what their requirements are.

I have Empire BC, switching to them was the best move I EVER made.
The requirements were WLS was "Medically Necessary" and a psyche Eval. no 6 month diet
 took them 9 days for an approval, for the VSG, and they called me at home to tell me I was approved and assign a personal counselor to me if I havd any other issues or questions.I never did talk her again afetr the approval was given
Of course there was a ton of pre-op testing but thats was more a requirement of the surgeon.
