Anthem Blue Cross - anyone had luck getting VSG approved?

Has anyone had luck getting BC to approve the VSG?
I"ve heard that BC Federal plan will approve it, but the individual and the group plans consider it "investigational"...not sure how it can be investigational and not at the same time.

Has anyone who's been denied coverage with BC for the VSG , has success with an appeal?
