Wow, Emily ... are you a role model or WHAT?! You're one strong sister, woman, and you've come so far. I know you have strength in you that will get you through the personal stuff as well as you're getting through the DS stuff.

I was going to answer the original post -- then I saw yours and was moved to answer it first. Being so close to what you are going through.
Death risk is a reality with any surgery -- you minimize the risks by making the best decisions you can. As you have done.
Our starting weights are almost identical -- I have 4 children ages 7 (twins) to 16 (I even have an older son 22). My DH and I thought a lot about the risks involved -- what would happen to the kids... if I were to die, or, because we travelled to Brazil, if we were to both die. We made logical decisions, revised/updated our wills -- asked dear friends if they would take all our children (and the home and everything else, of course), if something were to happen to us. They were honored -- we were relieved. We were especially relieved, because then we quit worrying about it.
I believe that our days are numbered. When it's our time to go, its our time. You are taking a minimal risk for a greater possible outcome. I know you want to be the best, healthiest Mom you can be. Your oldest son will be thrilled when Mom can go bike riding with him, or play catch or go to all his school functions -- because you have the energy and you WANT to go. He will be proud of you! I'm sure he is just worried about you.
This is not about being selfish or vain -- it's about taking this opportunity you have been given to be healthy and increase your lifespan!
