Keep thinking m'dear....And even better would be if you could come up with "real professional" citations to back up these good ideas. Bugs the hell out of me that WE are not "good enough" sources for this stuff, but the "professional" world has rules, so we gotta try to follow those rules where possible.
But if you're reading here and KNOW something, don't walk away cuz you don't have the citations to back it up. There are folks here on the board who are geniuses at finding that kind of thing. Post away and don't worry about that format/evidence thing. I'll keep asking for "proof," but that doesn't mean your input is invalid. I hope you'll be motivated to look at professional literature and to rethink and reformulate your statements to be more detailed and concise.

Very good points all! Now, how would you phrase these points in language that could go into a university level paper for presentation to professionals? Can you find professional literature support for your points? (I know, those questions suck, but those are the kinds of things that docs are trained to see. They tend to go blind, deaf and mute to regular folks and regular language. So we gotta work on this to make it, um, "professional" quality.)

I think you might be confusing psychologist and psychiatrist on the time issue.
When we talk about "qualified therapist," that's a term that is very nebulous. I don't know of any testing or licensing procedures/instruments that connect psychotherapy and bariatric medicine, though I'd love to hear about any such options that might exist worldwide. Both of those terms, "qualified" and "therapist" are not legally or ethically defined, much less together and wayyy less when we focus on bariatric medicine or psychology.
This means that we could participate in a totally pioneering endeavor, if we could work out "how" to do so, in contributing to developing input into a field of psychology that is, if not nonexistent, certainly undefined. In order to do this, we have to form our thinking to conform to a whole bunch of rules about content and presentation.
So, to go back to your post and shine a little spotlight on your comments: Define these terms: non-compliance issues, liars, disorders that would cause them to have a hard time coping with the post-op life.

If you could devise a DS-appropriate preop diet and nutrition education plan, what would it include? (Please include professional citations wherever possible--sorry to be a party pooper, but again, this is about preparing a good STRONG presentation for the pros, and there are rules to follow.)

Oooo yes. Can you come up with any currently existing (well, Google searchable) tests that might fit the bill? I've just returned from two-plus months with my sis and BIL, who love me and my cooking and wanted me there lol, but who were quite honest about stink issues. There were times when I chose to eat carby stuff and I farted a bunch and it STANK BAD. I lit candles and sprayed and went outside. They teased me but assured me that it was no worse than any of my siblings' (including my sisters') farts, except perhaps for having more power to linger.
Yes, I inflicted some carby fart stuff on them. They dealt with it with aplomb. My diet choices for my vacation were different from my diet choices for regular life when it's really not okay to choke people :-p. If it was really all that bad, they would have said so.
My hubby is blissfully lovingly unaware, unless I happen to be, um, very close in a gassy situation. So he's not the greatest monitor of stench.
But for the rest of my life? I think about what I eat. I chug Gas X if I eat carbs. I carry a little can of spray and a bottle of citrus essential oils (which are great in public restrooms). Frankly, I'm a hell of a lot more attentive to my potential bathroom odors than my fellow students are. I dunno what it might be like in the professional workplace cuz I have minimal exposure there. Ask me how many of my fellow students carry their own air freshener into public toilets *snort*.

I'll start with my own minimal explanation that I give to random strangers: "Most of my stomach was removed, but what's left is completely normal in function, just lots smaller. My small intestine was divided approximately in half and rerouted so that stomach contents go down one side and digestive enzymes go down the other side. These two sides meet at the bottom of what is, in my case, about 10% of the small intestine. That's where I absorb protein and fat, so that I have to eat a LOT of both of those to avoid malnutrition. I also have to take quite a few supplements to offset the malabsorption and get lab work done every three months."

You know I'm a big fan of therapy! Let's talk about what would define a "qualified therapist" in this context.
My personal thing is that I would prefer a psychologist (with apologies to the MSW/LCSW folks and acknowledging that I am pursuing the LPC--it's possibly a silly prejudice I have). Can we come up with a body of information/knowledge/help for this area?

Good points. Can you think of questions that you'd include on such a test? (Don't worry about the structure of the test, just about question content. We can all work together to edit the structure and phrasing here.)

Two of my especially delightful side dish discoveries: grilled potatoes and grilled zucchini.
Taters: Choose new taters with thin skins that are about the size of an extra large egg. Scrub em nicely. Toss em in olive oil with a little garlic. Throw em on the grill so they sear and get marks. Turn down the heat and close the lid and let 'em bake, checking them frequently.
Zukes: Maximum zuke size is about 8-10 inches. Clean 'em off, whack off the ends, slice in half lengthwise and each of them in half long ways. Brush with garlic infused olive oil. Throw 'em on the grill on high heat so they get grill marks. Turn down the heat, turn em over a couple times, and cook till they're as done as you like. Easy peasy.

Another fun grill thing is the kebab. Key here are a couple of things: 1) even sized bits and 2) good solid skewers.
Your meat needs to be actually a tiny bit BIGGER than your veggies. This is counterintuitive cuz with think of rare meat and cooked veggies, but it is nonetheless true, especially if your meat is beef. Marinate your meat o choice in precut hunks. Thread 'em all on the skewer. BTW it's totally honorable, especially as a DSer, to make skewers of all meat and other skewers of all veggies, plus it makes cooking issues easier.
Yummy marinade choice: Salad dressing of choice for everything. Guaranteed to suit everything on the grill. Creamy dressings are more likely to flare and scorch than translucent dressings, but both styles are doable. French dressing gives a particularly interesting finish because it's sweet tart and somewhere in between creamy and translucent.
Build your skewer being mindful that your veggie choices are likely to be oddly cooked unless you're an expert. If you're picky, refer back to advice to not mix meat and veggies, cuz meat doneness is less negotiable than veggie doneness for health reasons. Sear your skewers on at least two sides on very high heat. Turn the heat down and grill till the MEAT is appropriately done and to hell with veggie doneness. This sounds callous but you will acquire a feeling for how to get the meat and veggies done right.

My marinade of choice for all the steaks I was able to get out of sis's freezer:
One part lemon juice
One part soy sauce
Two parts beer (so a scant cup each of juice and sauce and a bottle of beer)
Globs of garlic and ginger
Good hard shakes each of five spice powder and red hot peppers

Aw **** thanks for paying attention to detail. Biggest risk at my house is that can of beer will be empty before it reaches the chicken's ass.
Today I bought my very own grill and had a friend assemble it, and presented my hubby with this gem. He whined about expense. He is banished to the couch and to canned food for a couple of days. I might just grill the first steak and eat it in front of him and tell him he gets PBJ for being a jackass. Said grill was 75% off, and I had someone assemble it dammit. I'm pissed.
Gotta love these marital moments LOL.

Dang. I don't get it. Ya know what? Try marinating hunk o beef for a good 24 hours first next time and see if that helps. Also, sear the beast off hard at very high heat on all sides before tossing it in the pot.

Wanna know what's WORSE? He has four half gallons of Candian Club hanging out there, from hunters who heard via the grapevine that it's the only whiskey he "likes." Thing is, "like" means a couple slugs per year LOL.
Worse yet? I sorted through the booze supply and found this lovely sounding cranberry schnapps. Nobody could remember where it came from. I poured a drink of the stuff and CHUNKS came out. From schnapps!! Who knew schnapps could get icky?! Trust me it did. Hurt my feelings horribly to pour it out.
Maybe if you come to MT next year and I tell him in advance that you're in no-booze world, he'll shake loose a bottle of the good stuff. He was awfully sorry he couldn't get together with the party animals this time, but haying demands, plus his crippled state, made it impossible. (Hopefully by next July he'll be dancing like a kid again--he's getting both hips replaced this fall, one in October, one in November.)

Let's brainstorm here. We know it's about more than IQ and BMI. In a perfect world, how would we structure a surgeon's practice to make it more likely that they'd only give a DS to a qualified candidate? What *defines* a qualified candidate?
What tools does a candidate need to succeed?
Let's work on this and make something of it that we can present to our surgeons, to ASMBS, etc. Think grassroots reform here!

If only, right? And a very few DS surgeons make a significant effort to reach this kind of goal....that of only giving a DS to people with the brains and mental health to make it work right. But that is too much to ask of just a surgeon. S/he would need a whole team, none of whom felt the slightest obligation to any bottom line other than the integrity of the procedure and an honest assessment of patient suitability. And dang, we ***** all the time here about surgeons who try to do just that.....
Thing is, they tend to do like Dr. Bonnani and Dr. Antavi.... can't spell his name.... and use an arbitrary number to weed people out. It isn't about size. How could this be addressed at the ASMBS level, I wonder? What kind of tools could be developed and put into surgeon's hands to make their lives easier?
