Mohawk Insurance?

I am looking for anyone who has Mohawk Insurance. Or anyone who has delt with them.

I know under the MedPlan policy, they had a weight loss surgery disclaimer and they would not even pay for it under medical necessity. I do believe they have new insurance now. I would advise contacting the utilization review and ask them what do you need to submit and to who to find out about pre determination of WLS. Good luck! Tara
