I was reading the reviews for Dr. Shina. On one of the reviews a lady who had medicaid said she had to have 6 months of paperwork from her primary doctor. I think the paperwork had to be about her weight loss tries and stuff like that. I have tried every diet available but I didnt have to go to the doctor for all of them. I only have few through the years involving the doctor. If any one has medicaid and knows what kind of paperwork I will be needing to get approved please let me know. Thanks Oh yea my consultation is on the 3rd of April.

I'm not sure if this is what Dr. Shina is looking for but that generally means they just want your medical records for the last 6 months. For Medicaid you don't need to have a doctor supervised diet. They just want to know the diets, products, etc you've used to try and lose weight.
