HELP!!!!!

Can anyone help me with getting my century club card?I don't know how to go about getting it.I am 150 pounds down from where I started 2 years ago.

I found this on the "help" menu... I hope it helps. But i did see on the main board someone was making them if you emailed them so maybe post there.
