Humana of KY
Anybody out there have Humana of KY PPO and has been approved? How long did it take? What hoops did you have to jump through? A friend is trying to get approved and is curious how long it takes. She has BMI of 45, osteoarthitis, and is a 5 year survivor of hodgekins lymphoma. She has also met with her family physician for the past 6 mos. related to obesity. Any help would be greayly appreciated! Thanks!
Carrie
I have that insurance, and am currently in the process of trying to get approved. The guidelines I was given are:
1. BMI of 40 or greater with one of the following: hypertension, type 2 diabetes, life-threatening cardiopulmonary problems such as sleep apnea, obesity hypoventilation syndrome, Pickwickian syndrome or obesity related cardiomyopathy, joint disease that is treatable if not for the obesity.
2. Physician documented successful attempt with non-surgical medically supervised weight reduction for at least 6 continuous months in the prior 12 months.
3. Eighteen years of age or older
4. Psychological evaluation
5. In good health except for obesity associated co-morbid conditions
6. Gastric bypass and Lap-band procedures are covered.
I am also having to do a three month pre-op program with the surgeon's office as part of the pre-approval stuff. I don't know how long approval takes, but I have three people with whom I have worked that had their surgery approved very quickly. I hope it works out as well for your friend and for me! I am planning on having my surgery in Nashville but the surgeon is a participating provider...Good luck, Kathy
