I just received this email from my insurance company: Dear Ms. XXX, Thank you for your inquiry regarding your Humana health plan. Our records show that Dr. Lusco is a participating provider at the three following locations: . 4402 Churchman Ave . 2355 Poplar Level RD. . 1903 W Hebron LN However, I called Dr. Lusco's office and they stated that Louisville Surgical Associates have a contract with Humana but they do not have a contract with Humana under Louisville Bariatric Associates and for this reason, for bariatric services, they are an out of network provider for your PPO policy. You may change providers, just keep in mind that the authorization we have on file, must be corrected to show the information from the new providers in order to be still a valid authorization.
