6 month weight loss program
My insurance requires the 6 month, Dr. supervised, weight loss program. I am going to call my Dr. today and set up an appointment with him. Is there anything else I need to do right now, or do I wait until the 6 months is up? I am planning on attending a seminar for the lap-band in July.

check with the weight loss team, and see if you have to also see your family dr. once a month for the 6 months also to talk about weight loss with him. I was suppose to and missed a month and had to go 3 more months, I don't remember being told that I had to see him once a month and found out at a meeting. then it was to late. so it took me longer to have surgery.
Thank you for the reply. I didn't realize there was a specific form that had to be filled out by the Dr. each month. Guess I should find out about that. I have not spoken to a Surgeon yet, did you just call Norton's to get the form for your family Dr.? Sorry for all the questions, this is all new to me, and I don't want to skip any steps.

from someone who has jumped thru hoops left and right MAKE CERTAIN you document everything...E-V-E-R-Y-T-H-I-N-G! I have been told too many conflicting ideas that I cannot even beging to tell you. The best thing is to cover your back by writing everything down- complete with dates, phone numbers and contact peoples phone numbers or faxes. That way if you need to refer back to something you will have it. I has helped me.
Hold on to the info in a safe place and keep it til you are done. Maybe in your weight loss journal.
Some docs wont see you until everything is completed- ie. 6 month diet, psyc and nut, cardio, pulminary...

If you are getting it done at Nortons it is in their packet. You can either get a packet from them or print one off thier website. They also hand them out at the seminar but if you are already seeing your DR you should go ahead and get it.

According to my surgeon you should make sure that the Dr. that is supervising your 6 month diet puts the following info in their office visit notes for you each month..... I did this and things went smoothly for me.
Date of visit (don't skip even one month)
BMI @ each visit
Current Weight and Height each visit
Discussion notes on how you are progressing with the diet/exercise plan
Documenting the fact that you at least discussed with your Dr. possibly using WL medications (you don't have to actually take them or have them prescribed just discuss it.)
What is the name of the specific diet they have you doing...counting calories, Atkins, etc.
Discuss the exercise plan you will also be on and your compliance at every visit
Document any other co-morbities you have i.e. sleep apnea, diabetes, high blood pressure
Showing a diagnosis code of morbid obeseity ICD-9 278.01
and finally....check with your insurance provider because some of them require you to actually lose a certain amount of weight during this process for it to be approved. I know, I know my head was spinning too--- so much to remember.
