I have Dr.Tanner listed in my Anthem Medicare PPO plan book, I was wondering if you'd mind telling me about what money amts his office and the program required from you before approval and prior to surgery date, I don't know if I am required to come up with a lg amt of money or not and I am not having much luck finding out a whole lot of needed info from my plan, Velma
