Nicole - I didn't forget you! I took me until last night to find where I filed the paperwork! I used Dr. Syed Rizvi at Harford Memorial - I can't PM you with the number from work, but if you send me one, I'll respond. He was very good and very geared towards people who have had WLS. He's got offices in Havre de Grace, Bel Air and Towson.
