Anyone w/ Activa Benefits in Gr. Rapids ins?

Hello, I'm wrapping up my 6 mos. supervised diet w/ my pcp in May and I was wondering if anyone out there has had experience w/ Activa Benefits out of Grand Rapids, MI? They had a whole list of criteria I have to meet. Co-morbidities, last 5 yrs. of records indicating that my weight has been significant, high BMI of 40 or more, etc., exercise, (joined a gym and work out regularly, does this count?, also saw a personal trainer about 4 times to get me started??), etc. The question I guess is are they difficult to get approval from? In anyone's experience? Also, I have to meet EACH one of the following criteria, there's a list, I will only ask for advice on 2. (1) was supervised & monitored by a physician working in cooperation w/ dietician and/or nutritionists & documented in medical records by a physician who does not perform the bariatric surgery. (2) Behavior modification or behavioral health intervention. I didn't see a nutritionist or dietician I just followed WW program and my Dr.s advice on the South Beach diet plans. Does this count? As far as behavior modification what are they looking for here? I attend TOPS meetings regularly and we discuss ways to eat and exercise and live a more healthy lifestyle?? Sorry to drag on, I'm just concerned that they are going to try and deny and I'll have to appeal, blah, blah, and I will if I have to. Just curious if anyone's had experience w/ them. Thanks so much for any advice or input in all this. I'm so ready for a new life and waiting has been so stressful! I want to live again, soon!
