Blue Cross/Blue Shield PPO of Michigan
I have BCBS PPO MI, and was approved and in surgery within 2 weeks. I provided a spreadsheet listing all of my dieting attempts in the last 3 years. I also had a nut class and a phys evaul on the same day. I was shocked how fast everything went! Now, I just had plastics and they covered the complete TT! I paid extra for BL/BA and anchor cut. I am very happy with my outcomes of both surgeries!

I'm three months out from my surgery. I have BCBS PPO and it was a breeze. Document all of your weight loss attempts in a spreadsheet and any time you spoke with a doctor about your weight loss. Also show any diet or exercise programs you have tried, indicate your success or lack there of.
I went to MMPC in Grand Rapids, I live in Lansing and there are PLENTY of places to get the surgery. MMPC is a center of excellence and I have a feeling that they have a MUCH easier time getting patients approved. I know for a FACT that NO ONE contacted my primary care physician, nor any of my documented weight loss attempts.
If you can make the drive, go to MMPC, they schedule all appointments in a single day. I believe that I only needed to go to the office three times prior to surgery.
