Letter from PCP for insurance approval

Does anyone know or can tell me what the important facts that BCBS wants in there letters from the PCP to get aproval for the surgery. And does anyone have any suggestions so I can get approbed right away. My PCP is apposed to WLS for anyone and is not cooperation. Thanks for any help you send my way. i want to get this approved asap. thanks Patti

My surgeon gave me a sample letter to take to my PCP. I have BCBS PPO. The letter has to state that you have had 12 CONSECUTIVE months on a medically supervised weight loss program within the past fours years with little or no results. They need to list the things you are being treated for (high blood pressure, diabetes, etc.). They should say you are a good candidate for the surgery in their opinion. That pretty much sums it up. The sample letter I have is short and to the point. Good luck!

I have bcbs ppo and the letter my doc wrote is in my profile....I think the entry is dated March 13 or there about of 2006. BCBS does NOT do pre approvals, but your surgeons office should have enough experience to let you know what exactly they are looking for. I may also suggest seeeking out a PCP that is WLS friendly for a lot of reasons. first if you do not have his/her support your road may be long and difficult. secondly if at some point you have to stop seeing your surgeon for follow up care, you need/want a doc that can /will do what you need as far as "monitoring" (ie blood work etc).
