Many doctors/PCP's are busy.  It might take time but I would request a letter of recommendation and let him/her know that you have a deadline that you need it by and is there any possible way that it can be done by then?  Simply ask.  And stay on top of it.  Call maybe now and then to check and see if it's been written. Talk to him.  Ask him.  It's what I would do.

Just ask. Mine had a letter of recomendation the same day she told me I need this surgery or I would die from my obesity related problems.  It took an entire year to get final approval and have the surgery. I got ot and I'm happy ~so don't give up no matter what ok. Best wishes to you. God bless, Annette

Call and tell him you need one.  I wouldn't call and ask necessarily, but rather let him know that this is what you need for him to do for his patient.  I would give him some time to write it (though my letter was written by one of his medical assistants and he just signed it!) and just stay on top of it.  Maybe call and check the status after a week, and if necessary make an appointment with him.  That is actually how I got my letter!
