Weigh****chers documentation?

Hi there, I am wondering if anyone knows exactly what type of documentation is needed for blue cross blue shield of michigan so I can be approved for surgery.  The only thing I need is a 12 consecutive month medically supervised diet, and Weigh****chers does qualify for this.   My records from my pcp only support me for the past 4 months.  I have been a member to Weigh****chers but what is it from them that they need.  I want to make sure all my ducks are in a row.  So if you can help Thanks - Patti.

More than likely you are also going to need a 3-5 yr weight history which documents what diets you are on and wt loss/gain, psych eval, letter from your PCP recommending the surgery which needs to state you have been on a medically supervised diet for 12 consecutive months. I logged into my insurance web sites message centere and asked them exactly what is required and they sent me a reply back 1-2 days later.  I just recently received approval and thats what I needed. I do not have any major comorbities. I am otherwise pretty healthy except for being overweight all my life. I hope this helps. Good luck with your approval.

I have BCBS of MI too.  When you went to Weigh****chers they give you your weigh-in booklet, you know, the one that is folded up with the date stamp and your weight each week?  I had kept these for years!!!  Dating back to 1999 I had them, so I just brought those with me to Dr. Kemmeter's office.  That was sufficient for BCBS.  Well, that and a letter from my PCP filling in the months that I didn't do WW during the last 5 years.

They want 12 consecutive months out of the last 5 years.

Oh thankyou for your reply.  That is what I thought they might need but I wasnt sure.  I just hope I can find the ones I had.  If not I will figure it out one way or another.  Thanks again.

My weigh****chers records got me my surgery! I had my last 3-4 years of doctors records but your ww is an apporved suprivised diet you are right there. what you need is when you weigh in they keep track on your card the one that  stays at your meeting, after you complete 52 weeks they give it to you, that is what you need to have your surgeon's office make a copy of, send that with your doctors records.
