how specific was the documentation you provided? My PCP has written a letter that outlines my diet history for the last 10 years. But just to be on the safe side, I've started seeing him on a monthly basis for weigh ins, discussing diet, exercise etc. Thanks for your advice!

FiservHealth sent me a letter stating exactly what they needed for approval. I gave that to my surgeon's assistant and she took care of it and sent it in and I was approved. I know they wanted my height, weight, bone structure. My medical records showing how long I had been morbidly obese, 6 month diet documentation, a thyroid blood panel, some info on the surgeon, a letter from my PCP stating he supported me on this, I had a letter from my sleep apnea specialist stating that the surgery would benefit me, and some other things. I have to say that FiservHealth was very helpful with everything. I think the documentation the assistant sent to them for review was over 30 pages long. Good luck!

I feel for ya Shawn, that's exactly what I went through. I also have a PPOM coverage but mine is Activia. They gave me the exact run around when I asked them what it takes to get approved. Ohhh she rattled it all off, but unfortunately I "do not" know short hand. And she told me there was NO WAY she could send it too me in writing??? Mmm hmmmm I called my HR at work and she couldn't believe that's what they told me....well needless to say my Insurance Co. mailed it too my house in writing with in 3 day's so yeah.... stay on them they have to give you that information and in writing if that's what you choose.
I chose that because I want too be able to go back on it and say "Ummmm, that's not what you wrote me and I have it in writing in front me!!!" So hang in there girl and keep fighting it's your right too know!!

I followed up today and told them I was still expecting the guidelines in writing. I also had my HR benefit manager call them. She promised to follow up with them today, so I'm calling her tomorrow to see what she found out. I agree...I want to see it in black and white so I can hold them to it!

HR said that they were told it was in the mail. I called ABS back today and asked if they could fax it to me, they said my request was still be reviewed. They stated it was just received yesterday (a lie, I faxed it 1/11 at 9:30 am.). I'm still waiting....
Thanks for thinking about me! Now if I can just get all of my medical records sent to the surgeon...that's another battle all in it's self!
