BCBSM PPO PREAUTHORIZATION

The guideline for bcbsm ppo changed on 1/1/08. They no longer require 12 months. Regaredless of BMI. With BMI over 50 there is no waiting period. With one less than 50 there is 6 months only.

I finally got a copy of the requirement for the VSG from BC.  My policy is thru the state of Michigan as a retireed employee.  The policy states a BMI of 50 is required.  So now I just have to sit here and wait for BC to determine if they will approve me or not with a lower BMI.
