Medicaid requirements!?!

okay, so what are the requirements?  I have  been told different things by the insurance cord. .   I have completed the psych eval, have the PCP letter , met with the surgeon, and attended the nut classes. I have been told the requirements for the office have been met.  what else is there?  everytime I  call something else is added!

this is what I have went by, but I have medicare and medicaid.
It must be done at a center of excellence...

From looking at what you have done, it looks like you are covered, but you have to make sure you meet the BMI and the co-morbidities.  Someone told me once that they require a 6 month diet, but I find nothing on that when I looked for it.
