Lost my blood work requisition, Please help.

I am going for my nurse's appt tomorrow and I want to get my blood work a few hours before going so the nurse will have it. I am going crazy because I have lost my requisition. I have left a message at the clinic but I'm afraid they won't get it in time. Has this happened to anyone before and if so what did you do to get another copy?

I am thinking if I don't hear anything I will go an extra half hour prior to the time I was going to go and scoot into the clinic to ask for another copy.

Any advice or other suggestions you could give would be greatly appreciated.

I had this happen before and I called where I got the requisition and got them to fax another one  to the lab. That way the nurse got it on time. Good luck.

Thanks Ottawa 2020. I am pretty sure the requisition came from the WLC. I have left them a message but still no reply. I am going to keep trying to call in hopes that someone will pick up.

Yay, got a hold of Kate at the WLC and she is doing up another one. I just have to pick it up prior to going for my bloodwork!
