My Best friend has Unison.  I dont know if Unison required it or not, but her surgeon required her to do a 6 month supervised diet.  Her surgeon required her to loose 10% of her weight or else he wouldnt operate.  She complained ALOT  lol but she did it because she wanted the surgery.  I know that Unison did give her a hard time on approving it though.  She was denied twice but kept fighting and kept jumping thru their hoops.  She did finally get approved and her surgery is coming up in a few weeks.  Good luck on your journey.  Its all worth it in the end  :)
