I thought it was "the other end" (endoscopy).......  I agree that the gold standard is the biopsy, but the screening test can be a good (and less invasive) first step.  A lot of people who have negative bloodwork (like me - I have Dermatitis Herpetiformis) who are still having problems can go one step further and have a biopsy done.

Recently, some GI doctors have used a "pillcam" to scope the ENTIRE intestinal tract, and many early cases have been caught using that technique - a diagnosis is made from a visual determination if damage shows up farther down the tract than conventional tools can currently reach.

The OP might want to try a GFD to see if it helps their GERD at all - a screen for CD may or may not be helpful.  Some folks find relief from GI problems on a GFD whether they have CD or not.  It was just a suggestion -

I had forgotten to ask my surgeon if he would repair the hiatus hernia I had and when I mentioned that to my family doc she mentioned she spoke to him and he knows about it and he will NOT be repairing it at the same time. She explained that cuz i will still be overweight for a bit that the pressure from the belly fat could still push on the hernia and if it's stitched the pressure can cause the hernia to get forced through and casue worse damage.
I'm guessing mine is the type of hernia that slips up and down now and then, cuz I don't suffere from bad heartburn.. have chronic cough with it and random heart palpatations which are unnerving.
I'm going to speak with him before the surgery again when he pops in my room and discuss it a bit more. Cuz it worries me when I heard someone on here say if they don't repair it now they won't be able to after
