appeals with Medicare to get a revision?

 I was wondering if there is any one that knows how to go about getting a revision from lap band to vsg with Medicare and Blue Cross and Blue Shield?

First off the surgeon and the facility have to be part of Center of Excellence for Medicare to cover any bariatric surgery, the problem with the sleeve is that it has not had enough history to be covered by Medicare, the CPT code for the gastric sleeve is 43775 and is still considered contraversal. I am not sure if it is different by region but this is the policy for Illinois.
