I'm going through the same thing.  I had to do all the testing before insurance would even look at it.  It says on my policy that it is not approved.  The doctor's office called and they said if they could prove it was necessary, they would approve it.  I did all the tests and sent the paperwork in.  The doctor's office received a pre-determination letter last week and set my surgery for July 2nd.  I got a call 2 days ago that the office called to get a pre-certification and they said they had not approved me.  I have BC/BS of IL, but it is through Central States.  Apparently, BC/BS had done the pre-determination, but Central States said that they should have done it.  Now, when I call the insurance company, they say there is no way they will approve it.  Why did they pay for all the testing if they aren't paying for the surgery.  I can't afford it on my own, so if they don't approve it, I'm screwed!

wow! i  swear its rediculous! they sure know how to collect their payments every month and we pay out  the nose for this coverage.  Good luck to you!
