Oh boy do they ever lol! I'd say for your needs measure with your bra on just be sure to use the same bra. Now buying bras that's a whole another kettle of deflated water balloons.

Since my boobs resemble tube socks full of marbles, I measure my bust with my bra on.

I REALLY need to buy some new bras this week.  The braless view is very sad. BUT all stuffed in a bra not too bad.

I measure around the fullest part of my boobs without a bra, basically bc I hate wearing a bra and only do it when it's necessary (I work from home).  I don't have a lot of extra skin in that area, but they are sad & saggy from volume loss bc of weight loss & breastfeeding.  Can't wait to have them done :)

If you're doing it just to count inches lost, it doesn't matter where you measure as long as you consistently measure in the same spot.  Sizing for a bra is totally different.  Let the experts at the store do those measurements.
