OT: Tomoxifin question- anyone out there use it?

For now my oncologist has been prescribing me Tomoxifin until I can work out the logistics on future surgeries.  I have not had discernable side effects and he mentioned that this was fairly unusual.  A little later in the conversation he by happenstance menttioned that for gastric bypass patients they would have to up the dose due to malabsorption.  I know we are not bypass folks here, but do you think there is a chance we do not absorb meds to the same level as preop to a small degree?  I was warned that hot flashes, for example, are pretty typical- nothing.  Not that I am complaining, but I am curious.  I told my doc (again) about the sleeve so as a precaution he is going to pull labs to confirm that I am in the therapeutic range, but I am wondering if anyone else has any experience with this.

While the sleeve is not a ma-labsorptive surgery, per se, it does create a high pressure system with rapid gastric emptying. I don't know, but perhaps some medicines may be affected by the rapid emptying and don't get fully absorbed.

I ended up with a minor copper deficiency post VSG that I did not have prior to VSG. I have also had to take a double dose to get my numbers back in a range my bariatric MD (not surgeon) is comfortable with. I would not be at all surprised if science found in the future that the sleeve has some mild, mild mal absorption consequences.

As long are your labs are in the therapeutic range, I think you can relax. If anything, you might ask that your drug levels be monitored a little more frequently.

some drugs need stomach acids to modify them for them to work , we have a smaller amount of acid , and very little if you take a ppi

you may need a different drug or a different form of it , like iv , I know you don't want to here about iv drugs my hubby went thru 8 months of chemo a few years back , with some permanent side effects , but it is better than what would have happened with no treatment

have your oncologist , call your wls  surgeon for more info on the sleeve and how it effects drug, also talk to a pharmacist, the oncologist office should have one  that will know all about the drugs you take

hope it all works out good
