I had a specific amount each month I had to lose (3 months supervised) in order to be submitted for approval..Dr requirement, not insurance. I am not allowed to gain any weight from my lowest recorded weightloss weight prior to surgery, or it will be cancelled.

Just to clarify, you are talking about the supervised 2 to 6 months diet to qualify for surgery not, the pre-op diet that is usually 2 weeks and often liquid diet just before surgery, right?

My DD gained on her 3 month supervised diet and was approved as if she could lose she wouldn't need surgery.  I gained on my 6 month when I was banded.  I lost 60 pounds doing 9 months for the  sleeve and the doctor tried to talk me out of surgery.  She gave me the go ahead when I sited the times in my life I lost 60 to 80 pounds just to regain and gain even more.

BAND REMOVED 9-4-12-fought insurance to get sleeve and won! Sleeved 1/22/13! Five years out and trying to get that last 15 pounds back off.

I was not allowed to even meet the surgeon and schedule a date until I had lost 44 lbs in the supervised diet. Although the supervised diet was required by insurance, weight loss was not. It was the weight management clinic that had the 10% of your body weight lost requirement. It all depends on your surgeon's office/insurance!

I also had the same requirements set by my surgeon office as well.  I am now scheduled for surgery next week.  I have to go for a final pre surgery weigh in on Monday.  IF I have gained any weight since my lowest recorded weight (and insurance approval), the surgery will be canceled then and there and rescheduled.
