This is the letter from PCP for the insurance co.  Is there anything else that is needed?

To Whom it May Concern:

I am requesting prior authorization and clearance for the above referenced patient Tawanda Sawyer as a candidate for Vertical Sleeve Gastrectomy surgery.  This recommendation is based on her most recent office visit, which evaluated the following:

Medical Necessity for Patient:

• Weight 271lbs with a BMI of 41.2
• High cholesterol
• Slightly elevated high blood pressure

In my evaluation, I feel that my patient, Tawanda Sawyer would greatly benefit from this surgery.

 

You shouldn't need more that this. You clearly meet the criteria. Good luck!

Looks good to me. You might want to ask your surgeon's insurance consultant if that is enough information.

She said as long as it has the words "evaluation and clearance" in it, it should be fine.  Wish me luck!

I haven't finished my 3 month supervised diet but I have gotten my letter already and one thing my surgeon said had to be clearly stated in the letter is that I was medically cleared for surgery. You may want to make sure they put that in there before you send it.
