Insurance Question

I submitted my information to the surgeon's office yesterday.  I received an email stating i need to show 5 years of documention of being MO.  What exactly do I need?  Do I ask for copies of 5 years of medical records?  Thanks for the help!

 I would call and talk to your surgery offices tomorrow and see what that means

With Blue Cross Blue Shield PPO that means you need to have a copy of your medical records for 5 years showing that you have been obese for the last five years. I had sent in couples of all my doctors records from all my doctors highlighting my weight and any weight related problems.

From what I've seen posted on the boards...they are looking for a history of morbid obesity.  Some insurance companies that is just check-up records with documented weight.  But some insurance companies are very strict about what they will accept.  Your surgeon's office should have an insurance specialist that knows the ins and outs about requirements.  check with them!
