When I had a D&C I had to sign i believe such a paper but not for the RNY nor gallbladder. I know it is scary but trust in the Lord and you will be alright!

Don't worry sweetie!!!!! It will all be ok. I actually was ok when I signed my papers, only because I knew exactly what I wanted done and I wanted it in writing for my mom/family. That's what put me at ease.
