Topic: RE: possible internal hernia?
I am so glad to have found this posting. I had a lap RNY on 1/30/08 and began to develop gallbladderlike symptoms in May. The typical epigstric pain that radiated to my back with bloating and gas pain after eating the smallest thing. I immediately got an abdominal sonogram which confirmed several gallstones. After severalmonths of these intermitten episodes,I finally took the plunge and scheduled surgery to remove my gallbladder. I had gallbladder surgery on 10/8/08 and went home the same day. the net day, 10/9/08, I developed the same bloating, gassy feeling that eventually advanced to severe abdominal pain with nausea and vomiting. At the advice of my surgeon, I went to the ER where I was admitted. The pain was so excrutiating that Morphine and Toradol didn't  put a dent in the level of pain. By the next day, 10/10/08 I was climbing the walls in pain and begged to get further testing done as I was going out of my mind in pain. A CT scan was done and confirmed a large internal hernia with necrosis/strangulation of the Roux limb. I was rushed into the OR from the CT scan where my surgeon had to perform an open procedure due to the severity of my condition.  My doctor said I was an hour or two away from serious trouble.  I woke up with a naso-gastric tube as well as a gastric (PEG) tube in my old stomach.  I remained in the hospital for 10 days.  I am now one month post-op and have not had any solid food since 10/7/08. I am being fed through the gastric tube with liquid nutrition six times daily and have lost an additional 30lbs since I came home from the hospital three weeks ago.

I have recently attempted to initiate liquids and some soft food such as puddings and cream of wheat. Unfortunately when I eat some thicker consistencies I develop a reflux and vomit. I had an upper GI series just today which confirmed a stricture or narrowing of the intestine. So I will follow-up with my surgeon to see if it is just swelling that needs further time to heal or if the narrowing needs to be stretched through an endoscopy or if a revision of my surgery is needed.

It has helped me greatly to hear that others have had similar experiences so that we know we are not alone. I wish you all lots of luck and good health.

Oh that sounds like a complicated and painful course.  Were the ulcers in your old stomach or in the pouch?  Trying to understand if they were in the new pouch why did they take out your old stomach??  I have had an internal hernia and surgery for that plus the latest round of tests/exp lap that showed the old stomach inflammed by bile refluxing back into it.  I had a GT in too for a while and have lost about 12 lbs since this all started.  Not underweight yet but only have about 6lbs more to go before I hit that mark.  Really ironic.....who would have EVER thought I would have to worry about maintaining my weight????  Are you able to work Renee?  That has been the toughest part for me lately.  With the symptoms flaring up I have been out of work since I never know when they are going to escalate.

Hi Az,
I'm sorry to hear that you are having problems too. I had my WLS August 25th 2004 then I had my gall bladder out in September 2004. In April 2005 I got an ulcer in my pouch then in July I got a second ulcer in my pouch. September they both perforated and I had to have emergency open surgery  which resulted in a total gastrectomy.  I spent a good bit of 2005 and 2006 in and out of the hospital having numerous open surgeries for small bowel obstruction, scar tissue, and adhesion's. I ended up under weight at 94 pounds and had to have a feeding tube for a little while.  My last surgery was in 2007.  I have not been pain  or nausea free since my original surgery. I'm scheduled to have another surgery on the 19th of this month.  It's an exploratory surgery.  My Surgeon is going to try lap but can't guarantee it won't end up open. He will be looking for adhesion's and hernias. Well that's my story and hopefully this will be the last surgery and the end of my pain.
Thank you for asking and know that my complications were rare. I don't want to scare anyone.
I also hope you will be able to get some answers and relief soon.
Take care,
Renee

Sorry to hear you are having problems but you have come to the right place for support.  I have been struggling for a while now with unresolved abdominal pain, nausea, belching, bloating and severe gas.  Have had every test known to mankind and had exploratory lap on 9/22.  All they found is my old stomach is filled with bile and causing irritation and bleeding in the remnant.  No one can explain why or what to do to treat it and meanwhile I have not been able to work since September due to the symptoms.  I would like to know a little more about your complications, when did you have your surgery and why/when did they do a gastrectomy??

Julie...good to see you posting.  I was wondering how you were doing.  Sorry to hear about the GT falling out but you will have to find out soon or later how you will do without it.  I am hoping that you are okay and don't need it put back in again.  Lord knows it will be nice to be without it since you have had it in so long.  But I can relate to the fear of worrying if you will be able to tolerate having it out.  I ended up giving up my job this week.  I am not in any shape to return to work with episodes of double you over pain 2-3 times a week; ongoing nausea, etc so I had to finally give in and make the decision.  I still have vacation time so I am not going without pay but the constant worry of "when will I be able to go back to work" is not pressing on my anymore.  I also had a very high stress job and if these GI sx are related at all to stress or aggravated by stress I wanted to eliminate that factor too.  Scary to not know what is next....if the sx continue and the docs don't have any resolution then I may need to apply for disability.  The docs have sort of given up on me since they don't know what more to offer so I just sit and wait and hope for either the sx to abate or if they continue to escalate then I will have to end up back at the ER and hope that there is something they can see that can be fixed.  VERY DIFFICULT AND FRUSTRATING but I don't know what else to do at this point.

Malissa,

I hope your surgery goes well.  For some reason, my Dr. does not want to remove the remnant.  Even though it seems to be the entire problem to me.  I guess I don't really know about the biliary bypass?  Can you expand on that part so I understand?

I am now without a g-tube as of about an hour ago.  I awoke to a strange feeling, reached down and the tube was in my hand!!  The balloon had leaked out so nothing was holding it in.  I talked to the Dr., and he said I could either go quickly to ER and have it put back in or try going without it and see what happens.  I am choosing to see what happens right now.

I think there will be a problem because I still was draining 500 cc or more every day.  The drainage has been somewhat bloody also.

So I wait and see.  Hope I'm not in extreme pain this afternoon.  Kinda scary.............

Julie

Wow....I hope this helps you Malissa.  I will be watching patiently to see how you and Julie make out.  I am still hanging out there without much improvement in my symptoms.  Still out of work and don't know when I will be able to return.  I had 3 days without nausea but woke up today with it again and I just don't feel "right" taking the Reglan.  I am going to have to stop that today and it really isn't effectively controlling the nausea any how.  If I could get the nausea under better control I think that would help considerably.  Is the doctor that is currently treating you your original surgeon or a different one you found?  Good luck with your surgery this week and again I hope this is a solution for you!

Hey,
Sorry it has been so long since my last post and that both of you are still haveing trouble.  Unfortunately, I am scheduled for surgery this week.  THe plan is to remove the remnant stomach and to revise the biliary bypass to see if this will eliminate the symptoms.  The doc says it will be a rough surgery, but if it will take away the symptoms it will be worth it.  It does make sense to get rid of the non-functioning stomach, it is just laying there causing trouble.  I will keep both of you in my prayers and let you know how this works for me.  Unfortunately are symptoms are a rarity and the docs just don't know what to do with us.  I am being treated at Mayo clinic/hospital and they agree that this is a rare complication.
I will try to do better about keeping in touch
