Paperwork question, can anyone help?
Hello everyone, I am in the process of getting all of my paperwork together
and my PCP is asking for a detailed list of diets that i have tried and I want it to look professional and be in the right format does anyone have an example that I can look at or can you tell me where to find one I mean on a website or something. I saw one before on a website but can't remember where. Thanks so much!

I do not know if this will help but I have one that was in a surgeon's packet. I have it in PDF format so I can email it to you if you will send me an email.
I am old (53) and it was really impossible for me to remember all of the diets I had been on all through the years. So here are a couple of hints on getting the information for the diet history.

Compare notes with friends, family and co-workers. Many times people go on diets together. This will not only help you with the names of the diets but also with the dates.
Do an internet search for "diets" sometimes just seeing the list of names will jog your memory.
Go to Amazon or any of the large on-line bookstores and do a search for diet books. If you remember the name of the diet but you do not remember when you were on it -- look to see if there was a book. Most of us would go on a diet when it was popular and that would be when the book was fairly new -- so you could use the publication date of the book for your date.
Include all of your diets, even the ones you made up yourself. If you tried counting calories or cutting out soda or did anything different that you hoped would have an impact on your weight put it on your list.
Some people will include any exercise programs they were in on their diet list.
The last thing is just do it. Start filling in the blanks with the information you have. Go with what you have. Depending on what the list will be used for you may want to go over the list with someone from your surgeons office before you turn it in.
I just turned my paperwork in also... I did a ton of different pages, including 2 pages of diets... But what I also included (& Believe me it was tough) was 6 pages of pictures of ME... from birth, up through the years, Including dating, graduation, wedding, childbirth, etc etc, all the way up to the present... It was tough finding pictures of me since I've always been overweight (Severly Morbidly Obese is the proper term) and "camera Shy" Why get my picture taken, when I'm gonna hate my picture?? Anyway here is a list of diets (Meridia, Xenical, Redux/Phen-Phen, Weigh****chers, Physicians Weight Loss, Jenny Craig, NutriSystem, Slim Fast, Dexatrim, Nutritionist, Richard Simmons, Hydroxycut, Metabolife 356, Xenedrine RFA 1, St. John's Wort, Herbal Life, Green Tea, Diet Center, Chromium Supplements, Nestlé's Sweet Success, Ultra Slim Fast, Atkins Diet, South Beach, Phentermine, WellButrin XL, Accutrim, Stacker, Kathy Smith Exercise Video, Yoga, YMCA Gym, Jane Fonda Exercise Video, Calorad, Karate, & my last one The Ocean Whale Diet) that I tried and then next to each diet, I listed when I was on it (sometimes several times), how much I lost & gained, & who helped supervise it (Dr so&so, etc)... I also included an excel file printout of my measurements throughout the different times in my life I actually charted it... So what if there are a few years missing, the more info you can give the doctor to show that you have been OBESE (Fat, Overweight, UnPleasently Plump, you know the different words we like & don't like to use) for so long is more helpful to them... The more work you do for them, the less time it will take to get approved!!! Good Luck & God Bless....
