Kristy, I had my surgery done on Jan 11th of this year . I had a physcological Eval done and it was a out of pocket expense it cost me 561.00. The guidelines for medicaid is aa phscy eval, previous weight loss attempts for 6 months to year, primary ccare physcian letter stateing this is needed for whateveer help problems you maay have, education class, nutrional eval an recent ekg, and x-raay if you dont have one they will get it scheduled for you.
