How do I change surgery status and add a signature?

I have tried again and again but can't find the place to change my surgery status or find how to add a signature line. Anyone know??

Marc: Go to "My Account" at the very top of the page next to Sign out and Bookmarks. When it pulls up there will be 3 columns. The middle column will have surgery status where you can put in a date, etc. Also, under the help icon at the top, there is a couple of videos that go step by step on how to do "stuff". Hope this helps. Michael
