UPDATE ON PCP RFRL LETTER....
As Many of you know a few weeks ago I was worried about having to write my own PCP rerf.. letter... Many of you were very kind and sent suggestions on how this letter should be writen. I combine many of your suggestions and worked with the OBGYN that lives next door and came up with this.. I think it includes all the points that the surgions office came up with. I believe it covers all the bases.. What do you all think?? It has to go in on Tuesday.

LAST NAME is diagnosed as morbidly obese and all though currently in good general health, LAST NAME is fully aware of the health concerns facing someone of his size and his family medical history of Diabetes, Heart Disease, High Blood Pressure and other obesity related diseases. Over the years Mr. LAST NAME has made several attempts to loose his excess weight using traditional methods including; Weigh****chers, Jenny Craig, Nutra System, Susan Powder, Richard Simmons, Slim Fast, Metabolife, Atkins, Scarsdale, Opti Fast and others. This patient has experienced varying degrees of success with each of these plans but, to date he has not found any sustainable results using these traditional methods.
Mr. LAST NAME came to me in June of 2008 through a family referral seeking support as he was beginning the process of obtaining surgical intervention to aid him in his life long struggle over obesity. He was interested having the Laparoscopic Band procedure done and I offered my support. At the time of our first meeting Mr. LAST NAME weighed PRE DIET WEIGHT and had a BIM of PRE DIET. I think it looks great. A few minor typos - change "loose" to "lose" in the 2nd paragraph, add "in" into "he was interested", change "BIM" to "BMI" and fix the verb conflict in the 3rd paragraph by removing "has" from "has also demonstates".
Also, since I'm in marketing, I'd change "several" to "numerous" because I like to exagerate. Good luck with the letter an referral!
