Blood work spreadsheet?

Good afternoon all,
I remember seeing a spreadsheet outlining recommended blood work and the appropriate diagnostic codes but my searching proves that my memory is better than my computer skills. Can someone help a brother out?

I have a bloodwork spreadsheet that was sent to me by one of the guys on this board. It doesn't have diagnostic codes, but does have ranges reference ranges for the various tests.
