Ok whats next!
I assume you have had an appt. with your surgeon. I was given a folder with all my info, and a checklist of pre-op tests I needed. I had to see my PCP, a cardiologist for a stress test, a gastro. for an EGD, and a psychologist. I have heard others have also had to see a pulmonologist and had to have a sleep study test.
After that, all reports were sent to my surgeon, who wrote his own letter and submitted to insurance. Just make sure you have everything your insurance requests.

I would call the bariatric program coordinator (this is the surgeons office usually) each 'program' varies...but they should tell you the steps....
Some people need psych, endocrine, hematology, pulmonary, cardiology, gastro upper Gi clearance etc....
