I had BCBS HMO when I had my band done... I had to do...
Initial Consult
Psychologist Clearance
Nutritionist Clearance
Cardio Clearance whi*****luded a nuclear stress test
Pulmonary Clearance whi*****ludes a sleep study and chest X ray
Blood Work
Endoscopy
Some doctors also require an abdominal and leg scan to check for clots
Once I did all that (took me two weeks) I got my approval within days!
Initial Consult
Psychologist Clearance

Cardio Clearance whi*****luded a nuclear stress test
Pulmonary Clearance whi*****ludes a sleep study and chest X ray
Blood Work
Endoscopy
Some doctors also require an abdominal and leg scan to check for clots

First and foremost is to decide which surgery YOU want to have. Never let a surgeon tell you what you should have. They can give you suggestions and reasons but please do the research and make up your mind about that.
Dr. Pomp is one of the leading Dr.s in the bariatric world so you dont have to worry much about his stats. They are pretty good. But you can ask him if you want to know...
1. How many laproscopic to open procedures he has done
2. How many deaths in the last 5 years
3. How many (of the type of surgery you want) he has done
4. How many days in the hospital post surgery
5. Does your practice offer support groups and do they cost anything?
6. How many reversals/revisions have you had to do?
7. Will I get a date today or do you wait for the insurance approval to give a date for surgery?
Other questions will vary depending on the procedure you want. Some common ones...
LAP BAND
1. What size and company band do you use?
2. How is that decided?
RNY - Bypass
1. How big of a bypass would you recommend for someone my height and weight or is it a standard amount?
VSG - Sleeve
1. How big of a bougie do you use?

Hi all
Anyone has Empire BC/BS (ppo) and if so how long is the process. I called them and they werent all that helpful. But wanted to know what are the guidelines etc. Other than the BMI, what else do they require, and once you do all the test how long after is the approval?

I don't know where to get information.. I am 37 yrs old with BMI of 49.3 and 5'3. I've struggled with my weight all my life since I can remember. Yes I've lost weight before, always yo yo diets well I guess you have heard it all. But I am trying to get more information not sure what surgery is best or which one is suitable for me. I go for initial consultation with Dr. Pomp here in nyc. next friday I am sure he will guide me in right direction but i wanted to be prepared with questions, any feedback will be helpful and appreciated.

I see you have a date, so you must have been approved? Good for you. I have IH, too. Our plan is self-funded so i had a few extra hoops to jump through but it all worked out in the end. Have you been coming to the support group meetings? Dr. Pham and his staff are AWESOME!!!
There's a support group meeting this Tuesday night if you haven't already been to one (9/28). They are so helpful!

I am curious too, since I have IHA (Kaleida employees plan). My husband is currently just starting the WLS process and I am looking into it. When my husband called IHA to inquire as to if it was covered and the requirements he needed to meet for it to be covered, he discussed it with them to see if trying to get insurance approval would even be worthwhile and they told him that it should be covered. He has a BMI of 60 and comorbities of osteoarthritis, sleep apnea, and bipolar depression.

To follow-up on this - I just found out that this fund was set up by a group that raises money for families that have lost someone to a motorcycle accident. We are still waiting to hear about one that is local.
