I have UHC.....my plan is the CDHP. I had to meet requirements such as bmi over 40 with no comorbities, and have 5 yrs weight history, then had to do a six month supervised diet..... But after it was finished we submitted and I was approved.
still surreal to me, surgery is supposed to be Tuesday...

I think that UHC was very easy to work with. Again,every policy is different, but they were specific in what they wanted and quick to approve. I made certain that I and not the surgeon's office got everything ready and submitted so that I was certain it was complete (and besides the office staff in my surgeon's office are a bunch of dumba$$ and would have screwed it up, I'm certain). Be sure you understand what they want and get it to them. I was easily qualified due to my BMI and comorbidities. I don't really know how tough it might have been if I had been borderline. And remember that you must advocate for yourself. Certainly the surgeon's office wold like to get it approved, but they do not have the same interest in doing so as you do.
