If you have vomiting issues you can be more likely to have a B1 deficiency. Are you taking your B12 sublinguals? You could throw in a B complex (make sure it has B1). B Complex's smell and tast horrible, look for capsules. I used to take 2 B50 capsules

Everyone should be taking 100mg of vitamin B1 daily to keep levels normal and if there is a deficiency, you need to take more - I had a deficiency in B1 from about 4 months out and I have been taking 300mg daily every since and I am 3 1/2 years out now. B1 deficiency is very dangerous - it can cause nerve damage and can also cause cognitive damage - very similar to B12 deficiency. You need a lab test to determine - you want to be att the top of the level or over the top - if you are towards the bottom - you need to take between 200-300mg daily to get your levels up and continue at that level to maintain - there is no danger in being too high - you just pee out the excess.
Vitamin B12 - just like B1 - it is very dangerous to have a deficiency - you can develop nerve damage and if left too long can become permanent and also lead to congnitive problems. The normal range for a non surgical patient is 200-900 but for us, we need our levels to be up around 1200-1800 minimum and it can go higher - you just pee out the excess - but too low - below 500 puts you at risk for nerve damage. You said you are taking an injection once month but many need to take them as much as weekly or even 2 times per week, so you really need to get your level checked because once you have symptoms - you most likely have a severe deficiency in either B12 or B1 or both.
Also, copper is a possibily but it usually further out - you need 2 tests - copper and ceruloplasmin - if copper is high - you need the ceruloplasmin to determin if it is toxicity or wilsons disease and if it is low - it is due to a deficiency.
the other possibilty is if you have pain down your leg, it can start up in your thigh and radiated down the thigh into the calf and sometimes goes all the way to the foot - this can be a sign of spinal problems. This would require an MRI of the spine. I would see your PCP and ultimately either a pain management doctor or a neurologist.
