Wow, whoever posted this originally, I want to thank them. I filled out the online form which took all of maybe 5 minutes and received a phone call within 2 days stating I had been approved for premier protein shakes and bariatric multivitamins from my insurance. They deliver without mailing fees once a month to my door 2 cases of shakes and I don't know yet how the vitamins work but when I get them I will let you know.
I encourage you all to go to this website and fill the form out. They do all the work and really WORK to get your insurance to cover these items. In today's economy, every little bit of help really helps!

Well, it is not for everyone, I guess. I have had to put more info on sites for shopping than they ask for here and it is a secure website so I was ok with it.
Yes, they act as a go between you and your insurance. It was so seamless. I honestly never expected anything to come of it but they did all the work and kept me informed of the progress. I am happy.
I don't know about salary caps. You would have to ask them that.
I get my first delivery on Monday or Tuesday so I can give everyone more info after that but so far it has been an unexpectedly pleasant experience.
What brand of vitamins? I could not find that info on their website.

I received a call from them this week that they sent a form to my PCP to complete and he hasn't returned it asking for me to call. I haven't done it yet I do have additional questions to ask them.

Actually, they compound all medications. I am thinking of having my dog's compounded medication transferred to them if the price is right.
But now, I am really wondering as well. Soooo, I will keep everyone informed.
