This ad just popped up at the top of the obesity help site. It says psych evals flat $100 and done from the comfort of my home. Is this legit? Has anyone used them?

I had not used that. My surgeon required a phsyc eval from a list that he recommended. Which only makes sense since he was wanting to make sure of legit clearance before surgery.

I just looked at the site really quickly but it looks legit to me.  I am generally opposed to therapy work that is not done face to face but, i also don't text, so maybe I am just old fashioned.  That being said, your doctor may not accept this and may want you to be seen by someone in person.  There should be a decent therapist in your area who can do it.  That is also nice for you because then you have a relationship with someone close who might be able to offer support if you need it post op.  $100 is on the low end.  In my very rural area where things tend to cost less I think most folks are charging around $150.  If your surgeon is okay with it this might be an okay option for you.  Little word to the wise though, if you have mental health issues or a history of disordered eating, please go to a local therapist because you just don't want to get into this without adequate support.

I have not looked at the site, but I would worry about how licensing requirements might impact this particular service. There was a thread a while back dealing with psychotherapy over Skype, and about how mental health professionals have to have state licensure. If you're not in the same state as the therapist, they're violating the law by providing services to you.
