can you tell your insides are rearranged?

No.  I had minimal pain with my surgery, more of just a discomfort.  I stopped taking pain meds before I left the hospital.  I had my gallbladder out a couple of years before RNY, that surgery was much worse

I kept expecting to feel different too but other than feeling the restriction I can't feel the difference.

Yes everything feels different when you eat or drink and having things go directly into your bloodstream is totally different, i.e. sugar or alcohol.  Then there is the issue of needing to have any procedure done down your throat to your stomach, given you altered anatomy this is impossible, I had a blocked bile duct, which is a simple fix for normal people with an ercp (scope down the throat), for me it was ten days, two different hospitals and finally surgery through  my excluded stomach at a cost of 73k.
