The 6 month required physician diet?

I was wondering this for a while. I hope my question can be answered.
I have to hand in 6 months worth of reqiured physicans diet forms in a few months. I have four sheets so far. The first sheet was filled out in July. My question is  to make 6 month requirement complete will I need 6 sheets or 7 ?

My doctor completed the information for me and sent it to the surgeon.  You have to have 6.  One for each month.  I started in March and ended in August.  They don't have to be calendar months....just one per month.

 Mine is 6 but if you dont go to an apt within that month you will probably have to do 7 mos instead... Im down 2 4 to go!

Unless you have bcbsal! They require 7... The first visit doesn't count ;( just found out the hard way!

I had to have 7 visits, 4 weeks apart.  I started in February and completed my last appointment in August.  The first visit didn't count because it was the "start" and didn't show progress on your diet.  They also didn't allow you to move the appointments up to get through them sooner.  Had to be exactly 4 weeks apart, same day each month.

Humm.... You got me thinking Jennifer. gonna run and check that they are 4 weeks apart....pretty sure they are. I had a feeling that the first month doesn't count.
For me that will be the end of January.
Praying to have surgury done by mid-end of February.

 I'm with Jennifer.  1st one was the "start" and didn't count.  However, as long as my appts were in consecutive months, it didn't matter whether they were exactly 28/30 days apart.  In other words, I went Feb 3 and then March 12, April 9, etc.  I was approved on the first try and was sleeved Oct 19.  I have NYSHIP Empire BCBS/UHC, NY.  My bariatrics doctor had a special form my PCP used to document each visit.  It included my vitals, wt loss/gain from previous month and med concerns.  (Morbid obesity, high blood pressure, etc.)  BTW:  I NEVER gained during these 6 months and actually managed to lose 50 pounds before surgery.  At first I thought of these 6 months as an annoyance but used them to change my eating habits (low carb, high protein, travk food with myfitnesspal.com, no soda, etc) before surgey.  Good luck, TEE

wow thanks Jag, that was a wealth of info. I have a special form to be filled out by my primary too...
So far I lost 11lbs on the diet and gave up soda since June....
