My grandmother lived with us for five years, and had constant problems with constipation.  Her doctor told me that the young man's world revolved around sex, and to the same extent the old man's world revolved around his bowels.  I am afraid I have turned into an old man.

Go get some glycerine suppositories. The ones I got look like tiny enemas. It works quick and you will have some relief. Then take miralax every day. Keep on top of the issue and you will be ok.
If it is all hard in there and can't get out, this is the best way.

This issue has been a constant for me since surgery, i stopped taking collace because i seemed to need more and more, i still take fiber pills with every meal and have started eating 3-4 prunes a day and that has made a difference  for me..

If i get slightly constipated ill just add a couple more prunes...

Back to the fissure issue - these can be healed.  If you can work to make your stool softer that will help a lot.

I would buy some ointment with Calendula and/or Comfrey.  Something like this:  www.iherb.com/Hyland-s-Calendula-Off-1x-Homeopathic-Ointment-1-oz-30-g/3765

I assume you'll be getting something to numb the area.  I would use the Calendula ointment several times a day.  It promotes really rapid healing.

But until you get the stool soft enough to prevent so much stretching of the sphincter muscle, it will take a long time to heal.

Also, a general help for constipation is to put a small footstool in front of the toilet for your feet.  It relaxes the perineal floor and makes BM's a lot easier.

Thanks for the advice.  The dr. gave me lidocaine ointment and nifedipine ointment.  i was in so much pain tonight even after the ointments and baths (i'm assuming it's due to the exam today) that i ended up taking loritab again.  I know that will constipate me but i figure i already am constipated so i'd rather not be crying in pain all night and be able to get some sleep.  have to start taking colace but am afraid to swallow the pills this early out (plus i don't do well with swallowing pills and usually crush them).
